I expressly authorize, without reservation, the employer, its representatives, employees, or agents to contact and obtain information from all references, employers, public agencies, licensing authorities, and educational institutions and to otherwise verify the accuracy
of all information provided by me on this application, any attached resume, or resulting from a job interview. I hereby waive all rights and claims I may have regarding this employer, its agents, employees, representatives, for seeking, gathering, and using truthful and
non-defamatory information, in a lawful manner, in the employment process and all other persons, corporations, or organizations for furnishing such information about me.
I understand that this employer does not unlawfully discriminate in employment and no question on this application is used for the purpose of limiting or eliminating any applicant or group of applicants from employment; it is the intent of this employer to comply with all federal, state, or local laws governing the selection and hiring processes.
I understand that this application remains current (active) for 90 days from the date it is completed. At the conclusion of that time, the
application is retained for 2 years, however, if I have not heard from the employer and still wish to be considered for the position(s) to
which I applied for via the application it is necessary for me to complete and submit a new application.
If I am hired, I understand that I am subject to the at-will employment provisions of the state and I can resign with or without cause or
notification at anytime; in the same manner the employer can end my employment with or without cause or notification at anytime except as may be required by law. I understand that this application does not constitute an agreement or contract of employment and that no supervisor or representative can enter into such with any candidate or employee in writing or verbally. The only exception to
this would be an agreement or contract authorized and signed by the President of the company.
I understand that if hired I must be able to present, within 3 days of hire, proof of identity and legal authorization to work in the U.S. and that federal immigration laws require me to complete an I-9 form in this regard.
Lastly, I understand that any information on this application, on a provided resume, or supplied during the course of a hiring process
that is incomplete, false, or misrepresented will be sufficient cause to 1) eliminate me as a candidate for employment now or in the
future; 2) may result in my immediate discharge from employment regardless of the timing of when it is discovered.
